Parsons House Preston Hollow is a thriving assisted living and memory care community seeking a Full Time Assisted Living Director of Nursing.

The ALD is a professional who provides direct and indirect resident care services in accordance with the policies and procedures of the facility and licensure. He or she maintains relationships with residents’ families and physicians through direct communication on care issues. Directs care planning, care conferences & supervises Assisted Living staff.

Shift:

• 9am - 5pm M-F
• on call

PRIMARY JOB RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Administration (includes Safety):

• Responsible to develop, direct and monitor delivery of all AL programs and services and keeping up with licensure requirements and company policy and philosophy
• Manage:
• - 1) Assisted Living Staff
• - 2) Assessment, Care Plans, and ongoing Level of Care
• - 3) Medications, recordkeeping, ordering & safekeeping
• - 4) A/L billing
• - 5) Resident records
• - 6) Resident Care
• - 7) Assist in IL resident case management as necessary; maintain "at risk"resident meeting notes
• Promote positive relationships with residents, families, and or responsible parties
• Provide active support to AL Assistant and Memory Care Supervisor as needed
• Manage department safety programs including resident and AL staff safety
• Manage licensure compliance, including survey readiness binder for community
• Consult with the Home Office on operational procedures, forms, documentation, etc. necessary to maintain regulatory compliance
• Prepare month end packages for A/L department including business plan
• Manage LTC Insurance
• Oversee Incident Reports
• Prepare Incident Logs
• Meet new residents and provide “New Resident Orientation”
• RN Delegation

Bookkeeping:

• Prepare vouchers and code invoices
• Maintain department cumulative expense log
• Operate within budget parameters
• Other duties as required by business needs

Resident Care:

Assessments:

• Manage Service Plan Calendar and Service Plan
• Change of Condition Report
• Monitor Transfer Handle Rails quarterly
• Fall Risk Assessments
• Elopement Assessments
• Self-Medication assessments monthly

Resident Charts:

• Review, audit, and maintain Resident charts
• Thin files as needed
• Maintain TB records

Family:

• Notification of resident incidents, change of condition, unusual resident needs, Dr’s orders, appointments, etc.

Physician/3rd Party Providers:

• Notification of resident incidents, change of condition, unusual resident needs, Dr’s orders, appointments, etc.
• Ensure 3rd Party Providers are meeting licensure requirements
• Monitor care of 3rd Party Providers
• Ensure 3rd Party Providers have a current, signed agreement and are providing notes
• Meet with 3rd Party Providers regularly

Maintain the following schedules and lists:

• Flu shots/schedule
• TB Tests

What we require:

• At least 1-2 years experience in a senior living community (assisted living or nursing home)
• Must be able to clear nationwide background check, TB test and drug screening.
• Experience educating and training a team.

Pay: $72,800 - $93,600 BOE

Job Type: Full-time

Pay: $72,800.00 - $93,600.00 per year
