POSITION SUMMARY: This position supports the Care Coordination (CC) Program Manager to develop, implement, track, report, and audit productivity metrics for the CC teams and individuals within the Presbyterian Medical Services (PMS) CC Department. The strategic goals of the PMS CC department include facilitating care for patients, scheduling them to see their primary care providers (PCP) at PMS or at Clinically Integrated Network (CIN) partners / Federally Qualified Health Centers (FQHC), and to improve the rate of closure of Healthcare Effectiveness Data and Information Set (HEDIS) recommended quality care indicators for each patient they work with. Develops and implements processes to capture, collate, and report applicable and necessary data elements and metrics from PMS NextGen electronic health record (EHR), CIN partner FQHC EHRs, EagleDream Population Health, Managed Care Organization (MCO) online portals, and other available and appropriate data repositories. The analyst also oversees, maintains, and identifies trends and opportunities for improvement of CC data, databases, and workflows.

REQUIREMENTS:

EDUCATION AND/OR EXPERIENCE:

• Bachelor’s degree in Computer Science, Business Administration, Information Management, Systems Analysis or related field; OR

• Minimum two years’ experience in data analysis, business administration, project management, healthcare reimbursement, or other directly related field.

• Advanced Excel skills.

• Previous experience working with databases, collecting, and presenting data in a medical setting highly preferred.

• Proficiency using computers and desktop software systems, electronic medical records, or population health informatics.

• Working knowledge of medical terminology.

• Excellent problem-solving skills and ability to work effectively as a member of a team while being self-directed when necessary.
