Job description

Adult and Children Case Manager - Provide assessments, planning, linkage, advocacy and monitoring service to clients who have a serious emotional disturbance or chronic mental illness, while working closely with their families to maintain them in the least restrictive setting.

Bachelor's degree from an accredited college or university with emphasis in a recognized human service field and at least one year experience in mental health or related field.

Work in Duval and Nassau County

Job Types: Full-time, Part-time

Pay: $18.00 per hour
