We are seeking a highly motivated and experienced Home Health Case Manager to join our team. As a Home Health Case Manager, you will be responsible for managing patient care, coordinating with healthcare providers, and ensuring the delivery of high-quality healthcare services to our patients.

Responsibilities:

• Manage patient care and create care plans
• Coordinate with healthcare providers to ensure the provision of comprehensive care to patients
• Ensure the timely delivery of healthcare services
• Evaluate patient progress and adjust care plans accordingly
• Provide education and support to patients and their families
• Collaborate with other members of the healthcare team to ensure the provision of high-quality care

Qualifications:

At least 2 years of experience in home health or related field

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of home health regulations and guidelines

Job Types: Full-time, Part-time, Contract

Pay: $19.50 - $24.00 per hour

Schedule:

• Monday to Friday
