Job description

As a Case Manager you will provide assessments, evaluations and case management services for clients at risk for or diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders.

Description:

  • Conduct client assessments and evaluations and make recommendations.
  • Coordinate client services.
  • Develop and maintain referral sources.
  • Make referrals to appropriate resource agencies.
  • Review files and records.
  • Review client files for accreditation and licensure compliance.
  • Work with all assigned clients to maintain stability for treatment.
  • Work with clients in treatment or waiting treatment to effectively manage medical, financial, legal or other issues that are barriers to successful completion of the treatment episode.
  • Participate in clinical staffing and team meetings.
  • Monitor random urine drug screens.
  • Prepare and maintain clinical and program records.
  • Testify in court proceedings.
  • Assist in the training and education of other staff members, interns and students.

Education:

  • Undergraduate degree (required).
  • A minimum of 1 year of experience in a similar role.
  • Must be able to pass background and drug screen prior to start date.

Knowledge, Skills & Abilities:

  • Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to deal with individuals under stressful situations. Knowledge of crisis management techniques. Knowledge of infection control procedures. Knowledge of 12-step program, behavior modification, case management and addiction.

Job Type: Full-time (Non - Exempt).
