The Role
The primary role of the Outpatient Treatment Program Case Manager will be to work with a diverse team of professionals to ensure the highest quality of care for clients. This position coordinates referrals and services across systems to ensure all service needs including social, physical, and therapeutic goals are met.

Responsibilities:*
- Work with treatment team to create treatment plans to ensure efficacy of treatment
- Develop and maintain relationships with community partners in various community-based agencies
- Coordinate services across systems such as medical and psychiatric services, DFPS, housing, childcare, employment, education and/or training programs and other services
- Enter all services into medical records as required
- Perform all other duties as assigned
- Conduct educational presentations in the community as requested

Requirements:CREDENTIALS / EDUCATION

Bachelor's Degree in behavioral health or social services field
- Minimum of 2 years' experience providing intensive case management
- Minimum of 2 years' experience providing case management for clients with hx of mental health and substance use disorders
- LCDC Licensure in addition to bachelor's degree strongly preferred
- Bilingual candidates are highly recommended to apply.

EXPERIENCE / SKILL*
- Clear understanding of DSM V Diagnostic criteria for substance use disorders and familiarity with mental health disorders
- Must have training and experience with motivational interviewing, stages of change, relapse prevention, strength-based, trauma-informed, and culturally competent perspective
- Excellent organization skills and very detail oriented
- Highly flexible and able to adapt to changing job requirements
- Excellent written and verbal communication skills
- Able to work independently and within a team
- Above average computer skills: Microsoft Office (Excel, Outlook, and Word); CMBHS proficiency preferred
- Personal/reliable transportation
