Position Summary

Case Management is critical and essential to achieving the organization's mission. You will work directly with community members and utilize the Unite Us platform for case management and other internal and external systems to assess client needs, make referrals, and request required funding.

In this role, you will focus on the whole person, assessing needs, including behavioral, physical, psychosocial, and activities of daily living.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

• Work closely with internal Community Navigators and Community Ambassadors to identify community members in need.
• Work closely with local community-based organizations and partners: i.e., the Veterans Administration, V.P.A.N., CityHeART, Social Workers, and external referrers, to identify veteran community members in need.
• Provide intake into our Case Management system by phone (direct line provided), Zoom, or in person, collecting all demographic information and assessments to make community-based referrals.
• Be an advocate for Community Member basic and essential needs.
• Communicate with external organizations to provide additional information and coordinate to ensure the client obtains the necessary resources.
• Identify emergency needs and make appropriate referrals.
• Assist families with completing forms and documents for services.
• Provide community feedback to management and stakeholders.
• Attend internal meetings and community events as needed.
