Job Summary: The Clinical Nurse Care Manager will provide supervision to the clinical nursing team, assist with assessing patient care needs and implementing appropriate action, manage care management processes, work closely with nurses, providers, support staff, and patients and maintain compliance with organizational policies and state/federal regulations.
Responsible for working collaboratively with leaders, clinical staff, and interdisciplinary team to provide the highest quality outcomes for The Center's patients through a Care Management model. Must demonstrate a sincere dedication and loyalty to the mission, vision and core values of BRMC.
Responsibilities include the following. Other duties may be assigned:
Clinical Support Operations
- Demonstrates competency in clinical practice and knowledge of current standards of practices.
- Ability and willingness to perform direct and indirect clinical services and assist nursing and provider staff when needed.
- Possesses strong knowledge base of regulatory standards, including but not limited to Joint Commission, Patient Center Medical Home, and Meaningful Use. Under the direction of the Director of Nursing, maintains a state of readiness in accordance with regulations and standards; takes appropriate action to identify and correct gaps; and adheres to regulatory standards.
- Attends and participates in clinical nursing in-service educational programs and supervisory trainings.
- Must maintain current CPR and CE credits required for licensure.
Care Management Services
- Assists in the development/implementation of patient care plans following the Care Management model, identifies patient goals/approaches, resolves patient/family concerns, and summarizes concerns for Director of Nursing.
- Represents clinical staff on assigned care management, performance improvement, and other quality assurance committees as appropriate to improve quality and continuity of patient care.
- Helps conduct comprehensive assessments and analyzes data in order to determine efficacy of care management methods.
- Accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients within assigned clinical team.
Supervision
- Serves on clinical nursing selection team, evaluating prospective staff members and making recommendations as needed.
- Under the direction of the Director of Nursing, assures that orientation for clinical nurses is completed and in accordance with policy.
- Schedules clinical nursing staff for assigned team while maintaining staffing expectations for the entire clinical nursing department.
- Secures clinical nursing staff to cover vacancies in schedule due to staff absences. In the event this is not possible, responsibility for coverage lies with Clinical Nurse Care Manager.
- Assists the Director of Nursing with the completion of performance evaluations for all direct clinical nursing staff and other special projects as needed.
- Provides direction, encouragement and training to direct clinical nursing staff in the performance of their duties.
- Demonstrates teamwork, flexibility and creative problem-solving skills when dealing with others including the culturally diverse departmental staff, providers, patients, and/or families.
- Pursues own professional growth and development through obtaining/maintaining specialty certification, and seeks opportunities for growth.
