Heart of the Rockies Regional Medical Center in beautiful Salida, CO is seeking a FT - 40 hours per week, Clinical Pharmacist to join our team of dedicated healthcare professionals.

EDUCATION CLINICAL PHARMACIST

  • Must have a current Colorado Pharmacist license (RPh) and experience in both hospital and retail pharmacy.
  • Minimum of 3 years experience in hospital clinical setting setting.
  • Graduate of Doctorate of Pharmacy program preferred.

CERTIFICATIONS and/or LICENSING CLINICAL PHARMACIST

  • Current active license with the Colorado Department of Regulatory Agencies Colorado Board of Pharmacy.
  • Current BLS

ESSENTIAL DUTIES PER AREA OF RESPONSIBILITY

1. Reviews oncology orders and provides clinical pharmacy support per guidance or request of the oncology/infusion manager or providers.
a. Reviews, interprets and evaluates medication regimens to identify, prevent and resolve potential or actual medication-related problems.
b. Performs drug utilization reviews that include but are not limited to drug-drug, drug-food, drug-disease interactions, therapeutic drug monitoring, appropriate regimen and dose selection.
c. Performs patient medication counseling and teaching by providing drug information.
d. Works with oncology, infusion, pharmacy and any other relevant personnel to ensure continuity of care.
e. Works with oncology, infusion, pharmacy and any other relevant personnel to ensure maximization of pharmaceutical supplies and minimize pharmaceutical waste.
2. Works with the Director to develop and maintain clinical the antimicrobial stewardship program (ASP).
a. Implements and maintains the ASP in accordance with the core elements of a Hospital Antimicrobial Stewardship Program as recommended by the CDC and the Joint Commission. Duties include but are not limited to data collection, therapy recommendations, provider/pharmacy education, and protocol/pathway development.
b. Provides ASP specific formulary recommendations to the Direct of Pharmacy for P&T review.
3. Participates in daily clinical rounding for inpatients.
a. Makes relevant clinical recommendations to appropriate personnel.
b. Assists in transitions of care by communicating discharge medication plans with the outpatient pharmacy.
4. Performs initial and finial review of prescription orders for accuracy and appropriateness including: dose, frequency, side effects, interactions, and indications
5. Provides IV admixture services utilizing aseptic technique and ensuring medication stability and compatibility.
6. Will work staff shifts in the inpatient and outpatient pharmacy to fulfill staffing demands and/or remain competent in each department as to complete day to day activities.
7. Performs appropriate medication order computer entry and patient profile maintenance.
8. Performs all daily pharmacist on duty responsibilities in the absence of the Pharmacy Director or Outpatient Pharmacy Manager. Including but not limited to:
a. Quality audits
b. Preparedness for board inspections
c. Customer complaint resolution
d. Staff leadership
9. Performs other duties as assigned by the Director of Pharmacy or Outpatient Pharmacy Manager.

Job Type: Full-time

Pay: $54.98 - $68.70 per hour
