This is a full-time (Monday-Friday) position based in Daytona and will cover Daytona, St. Augustine, and Palm Coast.

The Clinical Pharmacist is responsible for chronic medication management at the center. This position reports to the Clinical Pharmacy Services Coordinator and may supervise pharmacy personnel including pharmacy residents and students.

POSITION RESPONSIBILITIES

• Performs medication reconciliations and transition of care services.
• Amend or create chronic medication management protocols.
• Identify and refer patients that would benefit from chronic medication management.
• Conduct chronic medication management services; including but not limited to anticoagulation, diabetes, smoking cessation, hypertension, Heart failure, dyslipidemia, COPD/asthma, weight loss, hepatitis C, PrEP and immunizations.
• Utilize various pharmacy systems to provide MTM services for the center.
• Conducts patient education/counseling necessary for pharmacotherapy.
• Identify new ways to positively impact HEDIS measures when possible.
• Serves as preceptor for pharmacy students and residents.
• Keeps medical staff informed of new medications and researches/answers questions related to drug therapy in a timely fashion.
• Fill in for the staff pharmacist in urgent/emergent situations.
• Any other duties as assigned.

EDUCATION AND/OR EXPERIENCE

• B.S. in Pharmacy or Doctorate in Pharmacy

AND

• Board certification from the Board of Pharmacies Specialties (Pharmacotherapy or Ambulatory Care preferred), or the ability to obtain within 18 months of hire.
• Completion of an ASHP Accredited PGY1 Residency with a focus in Ambulatory Care and/or an ASHP Accredited PGY2 Ambulatory Care Residency.

  Language Skills – Fluent Spanish helpful, but not required.

CERTIFICATES AND LICENSES

• Registered Pharmacist in the State of Florida
