CLINICAL TEAM MANAGER
Salary Range: $75,960 - $84,400 annually

POSITION OVERVIEW*
The Clinical Team Manager (CTM) is responsible for supervising a team of Masters level staff and Interns who offer support services for residents of HomeRise's supportive housing portfolio and program participants living in San Francisco. Services target residents/participants who have complex behavioral health issues and/or medical concerns (including aging in place). Following the principles of Harm Reduction and Trauma Informed Care, the CTM provides supervision per BBS requirements to Clinical Case Managers and Interns (and/or other supervision based on licensing parameters), assists with quality assurance for our San Francisco BHS Mental Health Medi-Cal contract, and supports achievement of agency-wide housing retention and Resident Services Department goals. Using evidence-based practices, HomeRise welcomes residents/participants into their new homes, while encouraging greater self-sufficiency across three drivers: Housing, Income, and Resourcefulness. HomeRises Case Management Model uses data to understand the resident/participant experience and includes intensive retention services; strength-based case management support and service planning; mental health, substance use and crisis intervention services; information and referral; and empowerment/community development for residents/participants. The CTM is part of the agency's Resident Services team and works in partnership with Housing Operations staff.
ESSENTIAL FUNCTIONS

Supervise and coach site-based Clinical Case Managers and/or other RSD staff, including employee performance evaluations, hiring, disciplinary actions and terminations
- Create and manage a student intern/trainee program
- Create and offer trainings to agency staff to improve clinical understanding and effectiveness of service provision; individually support RSD staff members to maximize skill development
- Support staff to determine and execute clinically appropriate individual therapeutic interventions to residents/participants; support and/or provide treatment services under Medi-Cal system of care
- Lead responder for crisis intervention, conflict resolution and de-escalation services
- Identify issues which interfere with the delivery of services and develops techniques to resolve them
- Support clinical determinations and facilitate 5150s as needed
- Complete documentation per agency policy and actively support contract reporting, data collection, record keeping, and chart reviews
- Facilitate case conferences and other meetings through presentation of cases, discussion of clinical issues, and exchange of information and ideas
- Ensures compliance with federal, state and local requirements
- Other duties as assigned

Supervise approximately three Clinical Case Managers (or alternate RSD staff) and several Bachelors or Masters level interns across several supportive housing or program sites

POSITION DETAILS*

Location: San Francisco, CA Primarily within the Tenderloin Neighborhood
- Status: Full-Time / Exempt (40 hours per week)
- Schedule: Monday-Friday 10 am to 6 pm with some evening hours; plus on-call support by phone
- Reports to: Senior Programs Director or Director of Resident Services
QUALIFICATIONS
*
Masters degree in a behavioral science (social work, counseling, psychology) or similar field such as Occupational Therapy
- Licensed (LCSW, MFT, LPCC, or Ph.D.) with CA Board of Behavioral Sciences or comparable license within CA
- Ability to provide supervision to ASWs, AMFTs and/or PCCIs under BBS guidelines strongly preferred
- Minimum one year of formal or two years of informal supervisory experience
- Minimum four years experience providing services to individuals or families with experiences of mental health, substance use and/or homelessness
- Demonstrated ability in providing effective clinical services within a context of harm-reduction practices and voluntary services, and in-depth knowledge of harm reduction practices
- Strong clinical observational and assessment skills
- Experience in engaging people, crisis intervention, conflict resolution, and meeting facilitation
- Strong communication and collaboration skills
- Experience in creating and conducting trainings
- Knowledge of supportive housing, housing first, trauma-informed care, motivational interviewing
- Demonstrated commitment to social justice and advocacy
- Excellent computer skills (Outlook, MS Word, Excel, Salesforce or other cloud based database).
- Strong written and verbal skills and the ability to communicate effectively with a diverse population of staff and residents/participants
- Desired qualification: Possession of current 5150 card
- Desired qualification: Fluency in Spanish/Mandarin/Cantonese/Russian a plus; bicultural a plus
