Small fiduciary firm seeks a case manager to support the health and welfare of adults with dementia, persistent mental illness, and/or in the I/DD community, and who are under guardianship. In this position, you will coordinate with care teams overseeing the medical care, housing, and welfare of our clients through monthly home visits, actively participating in their medical appointments, care conferences, and treatment plan meetings. Working with medical professionals, care providers, community members, friends, and family you will ensure our clients’ safety and well-being and maximize their independence and quality of life.

Working environment is approximately 30% on site working in your own private office and 70% in the field with clients. Caseloads to not exceed 30 clients.

Hybrid remote working and a more flexible work schedule is available after training period.

Minimum Qualifications:

  • At least three years of human services experience serving adults with dementia, persistent mental health diagnoses, and/or those in the I/DD community.
  • Reliable transportation, valid driver’s license, and proof of insurance

THE MOST COMPETITIVE CANDIDATE WILL HAVE:

  • A Bachelor’s or associate’s degree in a Behavioral or Social Science or in a closely related field.
  • Experience providing case management services for human/social service programs.
  • Experience preparing and maintaining written narrative documentation of the activities concerning individual client cases.
  • Experience working in a human/social services setting where you interviewed individuals and their supports in order to assess service needs.

  • Knowledge of trauma-informed practices that actively do not re-traumatize individuals and communities
  • Ability to remain calm and effective in stressful situations involving people demonstrating anxious, angry and/or uncooperative behavior
  • Knowledge of community resources
  • Knowledge of time tracking software
