The Home Care Connections Assistant will assist in all functions related to the JHHCG's Home Care Connections and possses an understanding of home care agencies, services and geographically regions served. The Home Care Connections Assistant will be responsible for processing patient referrals and assigning them to the appropriate Connection's Network Provider thereby assuring that the patient's needs are met in a timely and efficient manner. Other chief responsibilities will include, but not be limited to, entering patient information into Home Care Connections' database(s), verifying eligibility of benefit, and obtaining authorization from the appropriate Insurance Company.

Skills: Excellent interpersonal and organizational skills, with the ability to multi-task, including written and verbal communication skills; ability to keyboard; ability to be self-directed and confident wroking in relatively unstructured situations and offer excellence in customer service to HCC, patients, agencies, and home care staff.

Experience: Minimum one (1) year customer service experience required, health care setting preferred.
