Job description

We are seeking LPN's and RN's to service our patients throughout the low country (i.e., Charleston, Berkeley, and Dorchester County only). Nurses will be expected to service patients where they call home (i.e. residence, ALF, ILF, etc.) in care coordination with the provider within their assigned service area(s). The nurses are an extension of the provider. This is not a home health or hospice position; however, we do service patients that are receiving these services.

The following service areas are available:

• Summerville Areas
• Goose Creek/Hanahan Areas
• Mt. Pleasant/Huger/Awendaw Areas
• West Ashley, Ravenel, and Edisto Areas
• Moncks Corner, Cross, Ridgeville, Dorchester Areas
• The Islands (Kiawah, Daniel, Johns/James)
• North Charleston & Downtown Areas

Daily Responsibilities include but are not limited to:

• Assisting providers with any medical needs (i.e., orders, labs, vaccination administration, etc.)
• Traveling to patient homes (including ALF and ILF)
• Assessments
• Assessing patients with any acute needs and facilitating a visit with the assigned Provider.
• Skilled Nursing
• Coordinating and Facilitating Telehealth Visits with Providers (including specialty visits)
• Medication Reconciliations
• Chart Documentation
• Chart Audit Reviews/Updates
• Addressing Quality Measures
• Addressing Risk Factors
• Enrolling eligible patients into Chronic Care Management
• Capturing Chronic Care Management
• Strong Communication with assigned Care Team.
• Promptly assisting your assigned Care Team with any medical needs (i.e., administrative duties, orders, vaccine administrations, etc.)

This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday). Other incentives include a pay per visit, monthly bonus, and a car package benefit. This position includes many other company benefits!

• RN License (Preferred)

Willingness to travel:

• 75% (Preferred)
