Hospice of Western & Central Massachusetts provides special care for people who are terminally ill. This involves a team-oriented approach that addresses the medical, physical, socials, emotional, and spiritual needs of the patient. Hospice also provides support to the patient’s family or caregiver. Services provided at Hospice of Western & Central Massachusetts mainly includes home health aide, counseling, spiritual services, medical social services, medical supply services, nursing services, and physician services.

We are currently hiring a 40-hour RN Case manager to join our growing team! We are seeking those RNs who are passionate about providing end of life care to patients in a one-on-one environment where you can focus on the patient.

This position promotes autonomy in the field by giving you the opportunity to schedule patient appointments, while receiving support from office staff and clinical supervisors.

Requirements and Responsibilities:

Provides professional nursing care to assigned terminally ill patients.

· Records patient histories, performs various diagnostic tests, administers medications, and develops patient care plans in conjunction with other medical professionals.

· Performs examinations and prescribed procedures, operating and monitoring of medical equipment as necessary.

· Observes patients' condition and records observations in patient medical records.

· Assess symptoms at end of life and provides education and support to the patient and their family.

· Collaborates with practitioners and/or the Hospice Medical Director to ensure patient comfort.

· Ability to adapt to the changing visit needs of our patients

· Provides on-call coverage as scheduled

· Reports to the Clinical Director.

REQUIRED QUALIFICATIONS:

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current RN licensure in Massachusetts and CPR certification. Bachelor's degree, with one (1) year of home health or hospice care experience preferred. Management experience not required. Responsible for supervising home health aides. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Mileage reimbursement.
