SALARY INFORMATION: $22 per hour/ Pay Grade I
DUTIES: Under general supervision plans, directs, and implements activities for the Racial and Ethnic Approaches to Community Health (REACH) project for the Community Clinical Linkages program. Responsible for collaborating with REACH Program Coordinator, Ameri Corps VISTA, community partners, healthcare organizations, and community members to create a sustainable and efficient referral process linking DeKalb County residents to community-based health programs. Promotes the use of appropriate and locally available programs for individuals in the priority population (e.g., Diabetes Prevention Program, Chronic Disease Self-Management Program, tobacco cessation services, Food Nutrition Education Programs, Special Supplemental Nutrition Program for Women, Infants and Children, access to food banks, and assistance with housing or job training). Expands the use of health professionals such as community health workers, patient navigators, and pharmacists, to increase referrals of individuals in the priority populations(s) to appropriate and locally available health and preventive care programs. Other duties as assigned. (This is a grant funded position.)
MINIMUM QUALIFICATIONS: Master’s degree in Public Health Education from an accredited college or university OR Bachelor’s degree in Education, Social, Behavioral or Biological Sciences or a directly related field from an accredited college or university AND One year of experience in public health education.
PREFERRED QUALIFICATIONS: In addition to the minimum qualifications, preference will be given to applicants with experience in Public Health program management, policy, and systems. Experience in developing environmental change strategies to improve the health status of the community. Strong proficiency with Microsoft Office applications, specifically Word, Outlook, Excel, and internet applications. Bi-lingual (Spanish) proficiency preferred.
