DESCRIPTION:


The Community Resource Center is a unique department at SAC Health that aims to address health inequities within the community such as lack of access to care, food insecurity, homelessness and housing challenges, among many other issues, in the San Bernardino region. All staff of the department is required to have basic knowledge of the Social Determinants of health, and its impact in underserved communities. The Housing and Homelessness Specialist I (HHS I) will work with community organizations, government agencies, and property owners to secure living arrangements, negotiate rents, deposits and move-in terms on behalf of the clients. Performs other duties as assigned.

Location: G Street, San Bernardino, CA


ESSENTIAL FUNCTIONS AND DELIVERALBES:


  • Attends, as needed, all external housing stakeholder meetings, to assist the activities of the DHS program offices in coordination with other State agencies, Federal agencies, local government, non-governmental agencies, and other stakeholders.
  • Networks and maintain relationships with community agencies and providers relating to homelessness support. Active member of the Homeless Provider Network.
  • Maintains a schedule of regular street outreach stops, including mental health drop-in centers, campsites, and other consumer gathering places while remaining flexible to incorporate new street sites as they are presented.
  • Maintains current and general knowledge of programs/resources relating to homelessness and housing.
  • Researches and identifies potential funding opportunities directed at funding safe, affordable, and accessible housing for the target population.
  • Researches and identify emerging trends for the target population in housing services, programs, budgeting, and marketing, both statewide and nationally.
  • Collects and report program data, including but not limited to funders' required data.
  • Reaches out to area schools, agencies and other stakeholders to provide program information and to identify potential participants.
  • Works in collaboration with Coachella Valley Rescue Mission (CVRM) to meet the needs of the homeless community in that region
  • Other duties as assigned.


QUALIFICATIONS:


  • Education: High School Diploma or AA Degree in Human Services or a related field.
  • Requires a valid current California driver's license, current motor vehicle record and proof of auto insurance. Must maintain a good driving record and current auto insurance.
  • Minimum of 1 year of Human Services, Social Worker or equivalent experience including related internships. Experience working with homeless population preferred.


Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
