Care Coordinator / Support Specialist - Full time and Part time
The coordinator is responsible to work with the supervisor and other staff in developing, implementing, and evaluating individual care and programming. This individual will follow up directly with the program manager on any program or personnel issues as they impact the provision of services. This position requires staff to be able to transport Persons Served on a daily basis.
They are responsible to provide direct services to the individuals, including but, not limited to:
- Completing errands with Person Serve such as: grocery shopping, medical appointments, and community outings
- Financial budgeting and tracking
- Scheduling and handling medical appointments
- Other additional daily living activities the Person Served may have difficulty addressing independently.
- Basic computer skills
- Some education, training, and/or experience related to the field
- Previous experience working with individuals who have disabilities or mental health diagnosis.
- Valid driver's license and liability insurance
- High school diploma or GED
