• Support a diverse population of people with intellectual and/or developmental disabilities (IDD) and mental health diagnoses.
• Provide services within the community--in a person’s apartment, home or assisted living facility.
• Deliver training, education, and support in community-based activities targeted to maintain or increase independence. Examples may include but are not limited to: finding and accessing community resources, budgeting and financial management, household management, community engagement/socialization, health, safety and wellness.
• Collaborate with community service providers, such as: rental/housing managers, employment specialists, families, case managers, guardians/conservators, physicians/therapists, rep-payees, etc.

• Schedule is Monday through Friday, daytime hours

QUALIFICATIONS

• At least 18 years of age

• Valid driver’s license, auto insurance, and a reliable vehicle

• Ability to use a computer and type 40 wpm desired (you will be provided a laptop)

• Self-motivated with excellent communication and time management skills

• Preferred: 1-year experience working in Human Services.
