Immediate need for an RN with Long-Term Care experience for 13 weeks.
We have a census of about 18 20 residents and the shifts are 12 hour and would be 12 hour day (7a - 7p) or 12 hour night (7p - 7 a) shift, variable schedule.

REQUIREMENTS:
***Mandatory Vaccination is not required with proper Medical or Religious Exclusion forms (may be downloaded from the Oregon Health Authority website)
and submitted to Human Resources at Lake Health District.***
Due to COVID-19, we are requiring all new hires/contract employees to complete and submit to us a PCR or an RNA (nasal pharyngeal swab) within 7 days of their start date at our facility. They will also need to self-monitor and check their temperature daily for the 1st 14 days they are here.
**Must be currently licensed in Oregon upon contract acceptance.**
Pre-Service Dementia Care Training for Direct Care Staff-Online prior to contract start date.
Current CPR certification.
Long Term Care experience as a RN preferred. Acute Care RN experience helpful.
Must be able to pass the Oregon ORCHARDS background check, and LHD per employment drug screening to place the candidate on the Long Term Care Registry.

13 week contract, 36- 40 hours per week, 12 hour shifts, day shift and nights available, variable schedule, in Lakeview Gardens Long-Term Care facility. Occasional extra shifts, Small cottage units with census of 12 residents per unit. Responsible over CNA s . Our Long Term Care Cottages are currently separate from the the hospital, but within walking distance. Small friendly rural community with mile high location in south central Oregon bordered by California and Nevada. 3 hours from Medford or Bend Oregon and 4 hours from Reno and 6 hours from San Francisco and Portland .
**REMOTE LOCATED HOSPITAL*** ****Nearest international airport from our location is in Reno, NV which is 3.5-4 hours away.
