Position Description: ILP Coaches will be responsible for providing daily support to individuals with mild developmental disabilities or acquired brain injuries in their own place of residence and local community. Primary duties include teaching life skills to clients in the Independent Living Program on an individual and small-group basis, teaching Life Skills Workshops, transporting clients to various community locations, and responding to on-call emergencies.

Minimum Qualifications: High School Diploma or general education degree (GED) required, Bachelor’s degree preferred or 2 years’ experience in a social services capacity. Preferably someone with experience working with people with a disability. Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements. Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting. Must undergo and pass a drug screening and screening for Tuberculosis. Must have received or be willing to receive a minimum of 1 dose of COVID-19 vaccine by date of hire, with the requirement to be fully vaccinated within 30 days.

Essential Duties and Responsibilities:

  • Provide individual or small-group life skills training to clients in the Independent Living Program in order to facilitate successful management of daily tasks and routines. Life skills training may include, but is not limited to, household tasks essential to cleanliness and safety, meal preparation, and community access.
  • Collaborate with Case Managers to support and implement the long- and short-term goals outlined in each client’s Personal Achievement Plan.
  • Complete daily documentation of client sessions and provide Case Managers with oral or written reports on client’s progress and/or problems on a routine basis.
  • Report on the needs of the client, on changes in the client's condition, and on any observed problems that affect the client.
  • Co-facilitate Life Skills Workshops at Annandale Village or other community locations using established curricula.
  • Accompany clients, provide, or arrange transportation to clients to places outside of their residences for purposes such as employment, appointments, entertainment, exercise, recreation, shopping, and social activities.
  • Assume 24 hour on-call shifts on a rotating basis.
  • Attend all required and assigned trainings and meetings, including the following topics: understanding needs of populations served, basic meal preparation, provision of transportation services, housekeeping, home safety, handling emergencies in the home, and infection control.
  • Perform other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.
  • Perform other duties as assigned that may be necessary in the best interest of Annandale at Suwannee, Inc. to meet the patient care needs of the Villagers.

Physical and Sensory Requirements:

  • Ability to read, write, and speak English.
  • Ability to apply common sense understanding to carry out detailed written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
  • While performing the duties of this job, the employee is regularly required to use hands; use fingers; handle or feel; reach with hands and arms; talk; and hear. The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl. Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance). Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Employee is required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
