Intake Liaison

This position is onsite

Intake Liaison Responsibilities:

• Navigating through hospital EMRs to gather client medical information and the necessary documentation to process a referral
• Conduct screening and informational interviews with patients and their families as part of the admission process (.ie. verifying patient information- demographics, billing payors, the method for consents, etc.)
• Enter client-specific information and communication notes into the electronic medical record system
• Send electronic consents for care to patients/POAs
• Track clients throughout their stay under our care, ensuring their satisfaction, and follow-up as needed
• Utilizing in-house referral trackers to effectively communicate with our Internal teams regarding referrals/patient care

• Upholds a professional and respectful attitude in all aspects of the role
• Excels at multitasking and is detail orientated
• Self-starter individual

Intake Liaison Qualifications:

• Experience in a healthcare office, ideally a home health setting
• Knowledge of medical/healthcare and insurance terminology is highly preferred
• Experience with home care admissions/intake/registration is ideal
• Demonstrates effective verbal and written communication skills
• Comfortable navigating and utilizing electronic medical records and computer programs
