Intensive Care Coordinator

(Martinsville, VA)

Full time position is responsible for the implementation of the evidenced based model of High Fidelity Wrap Around. This model involves working with families who have children at risk of being placed outside the home. This position is designed to provide support to families by building, coordinating, and leading a multi-disciplinary team that welcomes service providers and natural supports alike. The position will utilize the team approach through monthly team meetings to build natural supports for the family, and prepare the family to transition to independence and self efficacy. Position will monitor implementation of this plan. Bachelor’s Degree in Human Services field preferred with minimum of 2 years experience working with children and families. Prefer experience or training in High Fidelity Wrap Around or case management.

Pay: $36,720.00 - $56,473.00 per year
