Pembroke Hospital is a 120-bed acute care, inpatient behavioral health facility located south of Boston, in Pembroke, MA. The Case Manager is a vital part of Pembroke Hospital’s partial program multidisciplinary treatment team. This is a Monday – Friday, 8:00am – 4:30pm, full time position. Pembroke’s Partial Hospitalization Program (PHP) provides short term, intensive group-based therapy to promote stabilization and skill development for adults and adolescents in the community.

Responsibilities

• Conduct biopsychosocial assessments
• Facilitate groups (strong emphasis on DBT/CBT skills)
• Conduct family therapy sessions
• Document clients’ treatment
• Complete diagnostic assessments
• Provide crisis interventions

Requirements

• Master’s Degree in counseling, social work or a related field
• LCSW, LICSW, LMFT or LMHC required (or eligible to receive within 90 days)
• Ability to multitask and shift responsibilities in a fast paced environment
• Strong interpersonal skills and organization skills
• Preferred prior behavioral health experience
