Pembroke Hospital is a 120-bed acute care, inpatient behavioral health facility located south of Boston, in Pembroke, MA. Situated on 26 acres, we offer inpatient and partial hospitalization treatment to teens, adults and older adults, and our services are designed to assess, stabilize, and treat patients by addressing their primary symptoms and problems. The Case Manager is a vital part of Pembroke Hospital’s partial program multidisciplinary treatment team. This is a Monday – Friday, 8:00am – 4:30pm, full time position. Pembroke’s Partial Hospitalization Program (PHP) provides short term, intensive group-based therapy to promote stabilization and skill development for adults and adolescents in the community.
Responsibilities
- Conduct biopsychosocial assessments
- Facilitate groups (strong emphasis on DBT/CBT skills)
- Conduct family therapy sessions
- Document clients’ treatment
- Complete diagnostic assessments
- Provide crisis interventions

Qualifications
Requirements
- Master’s Degree in counseling, social work or a related field
- LCSW, LICSW, LMFT or LMHC required (or eligible to receive within 90 days)
- Ability to multitask and shift responsibilities in a fast paced environment
- Strong interpersonal skills and organization skills
- Preferred prior behavioral health experience
