NYS Licensed Clinical Social Worker (LCSW)
We are seeking a full-time, New York State Licensed Social Worker (LCSW), to join our outpatient, mental health clinical team at Symphony Medical. This role will be a hybrid of telehealth and in-person services, at our outpatient office in Dobbs Ferry, Westchester County, NY. This job is from 9:00 am – 5:00 pm, with an expected 35 client visits per week.

Job Details:
- Conduct patient intake and assessment to identify emotional, social, and environmental barriers related to their treatment and/or life situation
- Ability to diagnose independently using DSM5
- Develop treatment plans to address mental, emotional, behavioral, developmental, and addictive disorders, impairments, and disabilities, reactions to illnesses, injuries, disabilities, and social problems
- Provide person-centered psychotherapy, both short- and long-term to individuals, couples, families, and groups
- The ability to safely manage patients in crisis
- Facilitate the care plan across a multidisciplinary care continuum, which may include interacting with consulting physicians, other health care providers, and third-party payers to facilitate the movement of the patient through the system.
- Collaborate with outside agencies, such as law enforcement, social service providers, partner organizations, and faith-based groups
- Familiarity with other local mental health and case management resources to make referrals as appropriate
- Timely documentation in the medical record in accordance with department policies and OMH guidelines.
- Maintain patient confidentiality
- Maintain a working knowledge of relevant medical/legal issues that impact patient care
- Perform other duties as assigned
SKILLS/REQUIREMENTS:
- Master's Degree and NYS license to practice mental health (LCSW)
- Excellent organizational and interpersonal skills
- Ability to engage patients
- Ability to work both collaboratively with others as well as independently
