Job Summary:
The Licensed Practical Nurse practices nursing care under the supervision or direction of a registered nurse and/or the House Supervisor. The directed scope of the LPN practice is in concert with the Oklahoma Nursing Practice Act, 2001.
Duties/Responsibilities:
- Greets patients/visitors to department Participates in and encourages actions that promote good public relations with patients, families, visitors, and the community. Respects patients’ rights to privacy and confidentiality of information. Provides direction to patients, families, and visitors in a courteous, precise manner with a positive tone
- Contributes to the assessment of the health status of the patients by providing a focused assessment as an appraisal of the patient’s status and the situation at hand. The LPN provides initial and ongoing data collection, and collaborates with the RN/House Supervisor on patient care and information. Participates in the evaluation of the patients’ responses to interventions while participating in the development and modification of the plan of care.
- By implementing the appropriate aspects of the plan of care the LPN provides direct care to patients assuring safety and comfort of the patient using established evidence-based procedures and department policies. Performs treatments as allowed by licensure and assists in treatments and procedures with attention to detail, accuracy, patient safety, and comfort. Takes and records vital signs of the patient in an accurate and precise manner. Uses safety precautions to meet individual needs of patients.
- Accurately prepares and gives medications and injections as authorized by Department of Nursing Policy and Procedure. Performs tasks in accordance with knowledge and skills acquired through education.
- Participates in patient and family teaching and instruction. Responsible for completion of assignments without interference to patient care. Able to perform in an accurate and precise manner in crises and emergency situations.
- Accurately communicates patient’s information to Registered Nurse, House Supervisor, and Medical Staff. Recognizes and responds to priorities in the patient care process. Recognizes change in patient condition or behavior and reports changes to Registered Nurse or House Supervisor.
- Communicates pertinent information regarding patient at verbal shift report using nursing care plan approach. Accurately communicates in writing through utilization of appropriate charting method and proper transcription of medical orders.
- Participates in sharing of knowledge through in-service, staff meetings, and assisting in orientation of new personnel. Makes sound judgments based on factual information; seeks guidance and direction, as necessary, for performance of duties.
- Provides proper notification for absence and tardiness, and is punctual at start of shift. Observes department uniform and wears identification badge. Demonstrates understanding of emergency procedures, fire drills, evacuation procedures, and internal/external disaster plans and procedures
- Any and all responsibilities and duties assigned.

- Ability to write legibly
- Ability to interact in a positive manner and communicate well with patients, families, co-workers, supervisor, and other employees
Education, Licenses, Certifications and Experience:
- Current LPN License
- CPR Certification
- ACLS Certification preferred
- Graduate of an accredited LPN program
- 1 year experience working with the frail and elderly population
