LPN WELLNESS DIRECTOR

Hamilton Place Personal Living, LLC is a 55 bed personal care home offering memory care and personal care services. We are looking for a Wellness Director to join our team! Applicant must be able to pass TB screen, Physical, and a background check.

Duties include but are not limited to:

• Plans and manages patient care according to each patient’s needs.
• Makes CNA/CMA monthly schedule
• Interviews patients and records their medical history and physical condition.
• Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.
• Provides routine care for patients.
• Administers medications

LPN license and 1-2 years recent nursing experience in Personal Home/Assisted Living community required.

As a condition of employment at Hamilton Place Personal Living, all Team Members are required to provide proof of COVID-19 vaccination or approved medical or religious exemption by date of hire.
