LVN - Field Nurse

Summary: Provides direct patient care to maintain highest levels of comfort and quality of life. Responsible for physical care of the patient under the direction of the physician and Registered Nurse.

Duties/Responsibilities:

• Provide care for patients and families according to an individualized plan of care coordinated by a Registered Nurse and in accordance with agency policies and regulations that define scope of practice.
• Assist patient in learning appropriate self-care techniques.
• Assist and supports the R.N. and/or physician in performing specialized duties related to end of life care.
• Report symptoms and changes in condition to physician and/or Patient Care Manager and/or Case Manager.
• Maintain accurate, up to date and timely documentation for patient records.
• Ensure problems, actions, outcomes & responses to care are clearly stated & readily available for review.
• Participate in “on call” rotation when R.N. performs triage for “on call.”
• Assist in providing services that foster the organization’s mission, values and goals.
• Participate in orientation of staff and continued education of medical students, nursing students, and other health care professionals including community as appropriate.
• Responsible for timely completion of paperwork required to meet State and Federal regulations. Documentation should be accurate and detailed to meet these regulations.
• Understands and adheres to organizational compliance and privacy programs.
• Assists in providing service that fosters the organization’s mission, values, vision and goals.
• Performs other duties as required and/or requested by management.

Qualifications:

• Graduate of an accredited L.V.N. school or through reciprocity.
• Currently licensed as an L.V.N. in the State of Texas.
• One-year experience as an L.V.N.
• Current certification in basic life support.
• Skilled in nursing practices; ability to cope with family emotional stress and tolerant of individual lifestyles.
• Good organizational skills required to handle a variety of tasks simultaneously.
• Good communications skills, both oral and written.
• Effective interpersonal skills.
• Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
• Ability to function as part of interdisciplinary team.
• Ability to travel to multi-work locations during workday.
• Must have reliable vehicle, valid Texas Driver’s License and liability insurance.
• Knowledge/proficiency in applicable Federal/State laws governing hospice licensure preferred.
