New start-up Behavioral Health Braylon Health Wellness Center is seeking a Family Nurse Practitioner for primary care services. The incumbent will provide primary health care services to individuals of all ages as a member of a health team assigned to the various health center(s). The incumbent will work both independently and collaboratively with the Health & Wellness clinical providers and the clinical medical providers. The incumbent will serve as a clinical supervisor and team member of Braylon Health and will oversee the direct medical/clinical services being provided by the agency, again, working collaboratively with a team of medical providers. The incumbent will receive administrative and programmatic support and supervision from the Clinical Services Director.

Essential Duties:

• Delivers direct primary medical care to patients of all ages at other clinical sites, developing therapeutic plans of care for prevention, management, and maintenance of optimum health for individuals.
• Collaborates and coordinates service delivery closely with other team members and/or medical providers.
• Assesses, diagnoses, treats, manages, evaluates, and monitors acute and chronic illnesses, administers prevention services, prenatal/postnatal services, and family planning and counsels psychologically challenged individuals.
• Orders, performs, interprets, evaluates, and reviews diagnostic tests and procedures.
• Prescribes and dispenses pharmaceuticals evaluates outcomes and dictates changes in management levels of therapeutics.
• Performs minor surgeries and procedures.
• Provides health education and counseling to those clients seeking family planning services promoting positive health behaviors and self-care skills.
• Makes referrals to health care providers within and outside of the Indian Health Service system, specialists, other clinics and community agencies.
• Maintains client records in a systematic Electronic Health Record (EHR) system.
• Serves as a consultant and collaborator to and with the health center team, clients and community health professionals.
• Directly oversees and supervises the clinical services of the nurses, aids and medical assistants as needed.
• Participates as a preceptor and role model for Nurse Practitioner students.
• May develop and supervise general operations of one or more satellite health center locations.
• Assists and instructs other health care employees in the management of client care.
• May be involved in interviewing and selection of health care employees.
• May be assigned “on call: responsibility for Health Department clients/clinics.

Ensures that client records are kept confidential consistent with the Health Department policies and procedures and HIPAA standards. Monitors HIPAA compliance within the work environment.

Qualifications:

· Education: Master’s Degree in Nursing and relevant Nurse Practitioner

· Experience/Basic Knowledge: 3-5 years of Nurse Practitioner/Provider.

· Required Licenses/Certification: Possession of a current license to practice as a registered nurse in the State of Ohio. Certification of Nurse Practitioner in area of specialty by the ANA or NACOG and possession of a license issued by the State of Ohio as a Nurse Practitioner. Maintain recertification every three years. CPR certification, CLIA training and may maintain HIV counselor certification.

Physical Requirements:

Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly.
