Responsibilities and Core Functions:
The Outreach Coordinator serves to shepherd patients through their experience with Tampa Family Health Centers (TFHC). Outreach Coordinator works to increase new patient and established patients’ engagement and care coordination.
Specific Authorities and Duties:
- Respects patients by recognizing their rights; maintaining confidentiality
- Patient outreach, communication and HEDIS Measures
- Microsoft Office (Word & Excel)
- EMR use, experience with patient telecom
- Chart Audits (Looking for screening completion, appointments, quality codes)
- Develop rapport with patients and schedule appointments
- Work with payors and representatives on quality initiatives as well as partnering with case management for non-compliant patients
- Recognize signs of engagements and identify patients with low to moderate levels of engagements
- Ensure patients are educated about their health and care coordination
- Maintains tracking sheets that demonstrate evidence of compliance with assignments
- Maintains patient care database by entering new information as it becomes available
- All other duties as assigned by supervisor and/or senior management team
Knowledge and Experience:
- Strong interpersonal, verbal and written communication skills
- Excellent patient engagement skills
- Proficient with using electronic health records.
- Proficient in using excel to track calls
Education, Certification, Training and License:
- Associates Degree and/or graduate of an accredited Medical Assistant program or Medical Technician program.
- Bilingual (English and Spanish) preferred
