Job Summary:

Housecalls for the Homeless is a street medicine program of Upstate Medical University that delivers medical, dental, vision care, psychiatric and addiction safety net services for men and woman experiencing homelessness in Onondaga County.

The Assistant Coordinator will support the Program Administrator and Physician in various functions including:

  • Schedule patient appointments with specialists and follow-through care
  • Coordinate follow up care (facilitate medication use with Upstate Hospital Pharmacy) for discharged patients on the streets, providing continuation of medical care and follow-up on referrals, especially for those with chronic illnesses to lessen risks of readmission
  • Engage with the Upstate Hospital Pharmacy to coordinate vaccinations
  • Coordinate delivery of medications, wound care supplies and equipment to patients at shelter and on the street
  • Order program supplies (maintain van and go-bag supplies, order med-kit refills)
  • Purchase medical supplies and equipment for patients, account for purchases and reimbursement
  • Assist in data collection and accurate record keeping

Minimum Qualifications:

High School or equivalent diploma; Qualifications:

  • Computer skills, proficient with MS Office applications (Word, Excel)
  • Knowledge of EPIC would be helpful, but not necessary
  • Strong organizational and time management skills
  • Excellent verbal and written skills
  • Must have a drivers license
