Patient Care Coordinator
RESPONSIBLE TO:
Patient Care Manager
HOURLY RATE RANGE: $
17.51- $20.14
Open to candidates residing in: CO, NM, WY, NV, OH, MO, KS, OK, PA, AZ
OR: applicants are eligible to work from the Denver, Colorado Admin Office.
Our Virtual Care Center operates: Monday-Friday 8am/9am-5/6pm and Saturday 8am-3pm.
SUMMARY:
Patient Care Coordinator will notify patients of abnormal test results and track the status of treatment plans and/or referrals recommended by PPRM clinicians and according to PPFA Medical Standards and Guidelines. Additionally, the Patient Care Coordinator may be responsible for providing patient navigation for patients in breast and medical follow-up under the guidance of PPRM clinicians and according to PPFA Medical Standards and Guidelines. This position is hourly, non-exempt.
QUALIFICATIONS:
- Must have excellent customer service, communication, typing and organizational skills.
- Knowledge of PPRM’s Medical Standards and Guidelines pertaining to follow up preferred.
- Knowledge of reproductive health care required.
- Experience working with sensitive patient information required.
- Experience delivering sensitive information or results to patients required.
- Strong computer skills and ability to navigate all Microsoft Suite applications required.
- Experience with NextGen or other electronic health records system is preferred.
- Reliable transportation and valid driver’s license.
ESSENTIAL FUNCTIONS:
- Electronically sends prescriptions to the pharmacy and documents appropriately in the Electronic Health Record (EHR) system.
- Supports Medical Records Department
- Adhere to procedures and requirements regarding the notification and follow-up of abnormal lab results as outlined in PPRM Medical Standards and Guidelines.
- Learn and stay current on all relevant protocol requirements.
- Coordinate, log and sort incoming abnormal lab results and consult with clinician regarding plans.
- Initiate telephone and written correspondence to patients regarding results and treatment plans as directed by PPRM clinicians.
- Effectively and accurately communicate abnormal laboratory and cytology results to patients and explain the clinician’s prescribed treatment plan.
- Provide nonjudgmental education to patients by offering information and/or referrals on reproductive and sexual health issues.
- Demonstrates excellent customer service to both internal and external customers by working towards a positive outcome with any problems encountered.
- Document accurately, appropriately, and efficiently all telephone interactions with patients and complete, accurate and timely medical record chart note documentation.
- Demonstrate ongoing proficiency in Electronic Health Records (EHR) and Electronic Plan Management (EPM) systems.
- Run NexGen reports and complete appropriate follow-up management.
- Assist with scheduling patient appointments as needed.
- Send correspondence to patients via mail.
- Maintain average call time and volume as defined by program guidelines.
- Serve as a resource to PPRM staff regarding follow-up information.
- Assist Quality Management team with creation and implementation of Electronic Medical Follow Up systems.
- Completes all responsibilities according to established protocols, policies and standard practices in the areas of customer service, quality assurance and regulatory compliance programs such as HIPAA (Health Information Portability & Accountability Act), OSHA (Occupational Safety & Health Administration) and CLIA (Clinical Laboratory Improvement Act).
- Assists in answering patient questions received via the Patient Portal.
- Participates in the training of licensed and non-licensed staff relating to care coordination duties.
- Assist patients in connecting to financial resources like WWC and subsidy funds.
- Provide support to ensure that Quality Management support staff is following all PPRM policies and procedures and the Medical Standards and Guidelines, for timely and accurate follow-up notification.
- Participates in abortion services (participation can include referrals, information, education, counseling, performing, scheduling, etc.).
- Other duties as assigned by supervisor.

COVID-19 Vaccine & Booster: PPRM requires that all employees are fully vaccinated and boosted as a condition of employment. An employee is considered fully vaccinated two weeks after receiving the second dose of a two-dose vaccine (Pfizer or Moderna) or one dose of a single- dose vaccination (Johnson and Johnson/Janssen). The COVID-19 vaccine booster shot is required by the first day of employment, or five months after receiving the final dose of the original vaccine series, whichever is later. Proof must be uploaded upon starting. You will upload a picture or pdf of your vaccination card(s) on your first day of orientation.
Flu Protocol: All employees working directly with patients or working in a health center will be required to have the flu shot or wear a mask during patient contact or while in the health center from October 1 – April 30 each year (Flu Season). The flu shot will be available to health center employees in the health center in addition to being able to seek the flu vaccine externally through the medical plan or reimbursement.
