Position Highlights:

• Work location: 1920 Libal St, Green Bay, WI 54301
• Engage patients in checking their vital signs, weight, and to answer questions related to their medical condition.
• Evaluate data to determine if any medical intervention is warranted, and outreach to the provider for orders.
• Follow-up care coordination via phone/video.
• Marketing of program to key stakeholders.
• Design and implement growth strategies.
• Collaborate with Remote Patient Monitoring programs regionally and nationally to determine best practice.
• Conduct auditing to ensure high quality care delivery. Routine evaluation of protocols and policies and procedures.
• Act as the liaison between Remote Patient Monitoring program and equipment vendor.
• Manage equipment inventory. We serve patients in portions of 9 counties in Northeast Wisconsin.
• Salary range is based on relevant years of experience.

Shift Hours:

Full-Time hours available . Hours 08-1630 during the work week.

Job Description:

Nurses use assessment skills to review and complete interventions related to patient clinical data (vital signs, weight, and health related questions). Nurses have the ability to engage with one patient at a time, build trusted patient relationships, and provide patient education and coaching to make a meaningful impact and improve patient outcomes.

Qualifications:

Bachelor of Science in Nursing (BSN) degree preferred.

Wisconsin RN license required.

Valid Wisconsin driver’s license in good standing and liability car insurance required.

Previous RN experience preferred.
