Job Summary:
The Pediatric Clinical ED Pharmacist provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The Pediatric Clinical ED Pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy.

- Sunday - Saturday
o Evening shift ED based position (every other weekend)

Basic Qualifications
- Bachelor's Degree (or equivalent experience) (Required)
- 3 years relevant experience plus Bachelor's Degree or PharmD plus three (3) years relevant experience or PharmD plus completion of ASHP - Accredited PGY1 residency training program. (Required)
- Licensed Pharmacist (Required)
- PGY1 and PGY2 training (Preferred)
- Per Pharmacy Department policy, all new hires are subject to pre-employment drug screening (post offer), as well as for cause and random drug screening during their tenure with Vanderbilt.
Key Responsibilities
- Responsible for code/trauma response, medication reconciliation, order verification, medication preparation and delivery, automated dispensing cabinet replenishment and optimization, patient education, ordering and drug information support for providers
- Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
- Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
- Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
- Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
- Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
- Collaboration and integration with pharmacy, nursing and medical staff, and excellent communication skills are essential.
