Buckeye Home Health Care is growing and needs to add to the team! We are currently looking for a Physical Therapist for north Dayton and south Dayton. Area of coverage will depend on candidate location.

We are looking for someone who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients. Buckeye Home Health Care strives for excellence and has one of the lowest hospitalization rates in the areas. The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.

Qualifications:

· Ability to perform therapy evaluations and OASIS documentation

· Work with other disciplines to give the best care needed for each patient.

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

· When appropriate teach and counsel family members about care and processes for disease/injuries.

· Attend orientation, meetings, and CEUs

Requirements:

· Computer skills; must be able to work within EMR (WellSky) system.

· Must have current PT Ohio license in good standing.

· Reliable auto and current auto insurance.

· Current CPR.

· New graduates are welcome to apply. We have a strong orientation program and are able to mentor new graduates in order to ensure a great experience in home health care
