Heritage Home Health Services is seeking a full-time/part-time PT who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.

Qualifications:

• Ability to perform therapy evaluations, routine visits, and OASIS documentation
• Work with other disciplines to give the best care needed for each patient.
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate teach and counsel family members about care and processes for disease/injuries.
• Attend orientation, meetings, and CEUs

Requirements

· Computer skills; must be able to work within EMR system.

· Must have current Indiana PT license in good standing.

· Reliable auto and current auto insurance.

· Current CPR.

· New graduates are welcome to apply. We have a strong orientation program and are able to mentor new graduates in order to ensure a great experience in home health care

Job Types: Full-time, Part-time

Pay: $90,000.00 - $100,000.00 per year
