Job description

Acton Medical Associates, a pediatric and adult medicine primary care practice, is seeking a Population Health Specialist. This position will support the population health and quality assurance department, which works to ensure all patients receive exemplary and consistent care by monitoring and analyzing patient care practices based on the current standard of care established by the National Quality Control Association and our providers.

In this role you will:

• Track health care data
• Compile and enter data into the EMR system
• Enter patient needed health screening orders into the EMR system
• Participate in quality improvement projects for the team and/or practice
• Participate in quality improvement projects related to the management of patient care
• Reassign documents to the appropriate clinical personnel for follow up
• Identify and recommend opportunities for improvement in the EMR system
• Communicate effectively with health care providers and outside agencies as needed

To succeed in this role you will need:

• Strong attention to detail
• Strong communication skills
• One to two years of clinical experience, preferably in a group practice setting
• An enjoyment of problem solving and a passion for high quality patient care
• Computer skills with basic spreadsheets, databases/EMRs, and word processing
• High school degree or equivalent

Pay: From $15.00 per hour
