Regional Logistic Center Patient Navigator. These are benefited positions at the Fairview Heights, Illinois health center. Starting pay is $17.25 an hour.

POSITION SUMMARY
The Regional Logistics Center (RLC) serving Reproductive Health Services of Planned Parenthood of the St. Louis Region is a multifaceted, patient centered system to ensure timely and convenient care for patients at Reproductive Health Services and Hope Clinic. A patient navigator is a community health professional that focuses on the abortion patient’s needs to access care on both an individual basis, and at a systems level. The patient navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels.
PHYSICAL WORK INTENSITY AND WORK ENVIRONMENT
Physical requirements require sitting at a desk 7-8 hours per day in front of computer terminal. Must be able to use hands for repetitive action on a daily basis. Lifting up to 20 pounds on an occasional basis and use of computer, telephone, calculator, laser printer, modem and fax on a daily basis. The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Responsibilities include, but are not limited to:
- Organizing schedules and managing appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences.
- Facilitating communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.
- Informing the patient on financial aspects of care and linking the patient to financial resources as needed.
- Working with the patient to identify logistical needs to access care and linking the patient to appropriate resources as needed.
- Working with the patient to identify other support services as needed, and providing appropriate linkages
- Accommodating interpretive language services when needed
- Facilitating pre and post-abortion visits and ensuring continuity of care and linkage to community resources
- Collecting, tracking, and reporting required program data
- Identifying and addressing existing gaps and opportunities within existing patient navigation processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational support
- Working with external stakeholders and organizations to build and strengthen relationships, systems, and processes that support access to abortion care
- Working with external vendors to facilitate access to key services, including but not limited to, transportation and lodging
- Working closely with affiliate and contact center staff, including but not limited to social workers, Justice Fund managers and counselors, contact center staff, health center managers, and others to create efficiencies and improve workflows for increased abortion access and improved patient experience
- Developing, managing, and advancing program goals and objectives for new and/or improved processes and systems that increase access to equitable abortion care
An effective patient navigator should have a demonstrated ability to quickly and effectively synthesize patient needs and liaise with providers and local resources to create a unique patient care plan. To that end, patient navigators should possess the following characteristics:

The nature of the work for a patient navigator will vary based on the organization and region in which they work. Some of the common day to day activities of the patient navigator may include scheduling appointments and/or referrals to other clinical care sites, coordinating health care activities of the patient, conducting financial intake and/or helping to secure funding for their abortion if necessary and organizing logistical and practical support if necessary. The patient navigator will help patients identify needs and connect patients to appropriate resources and supports. The navigator may be involved and coordinate with outside entities as well. The patient may have needs to arrange legal counsel, transportation and travel logistics, dependent care or anything else necessary to help them obtain their abortion that the patient navigator will assist with. The patient navigator collaborates with diverse organizations and personnel to ensure the needs of the patient are met. The ultimate goal of the patient navigator is to relieve as much of the logistical and practical burdens from the patient as possible, such that the path to obtaining high-quality, patient-centered abortion care is as clear and smooth as possible. As such, the role is also well positioned to identify, address, and improve the systems, structures and processes that create barriers to care at the level of the call center, health center, and ecosystem.
This position usually involves daytime work, but sometimes night and weekend hours may be expected. Patient navigators may work with anyone seeking abortion care that needs assistance. The patient navigator works closely with the health care providers, clinic staff, local and national abortion funds and other community organizations and volunteers.
QUALIFICATIONS:
- High school diploma or GED certificate required.
- Customer service experience, preferably in a medical setting.
- Knowledge of medical terminology a plus, but not required.
- Effective and efficient verbal and English writing skills.
- Able to function in a fast-pace environment, performing multiple functions.
- Computer literate with e-mail, and Microsoft programs.
- Detail-oriented and committed to accuracy and organization.
- Accepts constructive criticism and uses it towards self-improvement.
- Fiscal Accountability proven.
- Flexible, non-judgmental attitude towards individuals from diverse backgrounds.
RISK MANAGEMENT AND QUALITY ASSURANCE
- Documents correctly according to training in electronic medical records.
- Reports any unusual circumstances to Data Analytics and Special Projects Coordinator or other appropriate manager and follows procedures as outlined in manuals, memos, and other PP documents.
- Follows the most recent PPFA/PPSLRSWMO medical standards and guidelines and ensures the client is aware of individual follow up plans for medical service.
- Cognizant and compliant with policies and procedures as outlined in affiliate procedure manuals and in the PPSLRSWMO Manual of Medical Standards and Guidelines.
- Protects PPSLRSWMO and its assets from liability.
- Protects confidentiality of PPSLRSWMO clients, staff, volunteers, donors, Board, and all other areas of operation.
