- Effectively communicate/present BHH services to small and large groups.
- Demonstrate clinical competence in the nursing process.
- Facilitates and monitors the admission of consumers.
- Assess consumers' overall needs on admission and at 90 days. Re-assess as needed and for level change. Formulate and participate in Individualized Rehabilitation Plan.
- Consult with treating psychiatrist, partial care program, and medical physicians as needed to provide and facilitate appropriate treatment
- Collaborate with team members to develop and implement Individualized Rehabilitation Plan
- To Ensure Consumers receive the necessary monitoring, education, and support for any physical or behavioral health issues in areas of Medication management, diabetes management, healthy meal preparation, and activities of daily living.
- Delivers services that achieve quality, utilization, outcome, and financial sustainability.
- Responsible for ensuring that services comply with all licensing, regulatory, accreditation, and ESNJ policies and procedures.
- Participates in the implementation of new ideas and knowledge that improves services and operations.

- Provide face-to-face visits every 90 days following the initial comprehensive nursing assessment review consumer’s clinical condition which shall ensure that services being provided are consistent with the consumer’s individualized rehabilitation plan.
- Indicate any changes needed in treatment approaches in the Individualized Rehabilitation Plan.
- Teach Medication Management ESNJ Procedure No. MED01P to residential staff.
- Conduct Quarterly Medication audits.
- Provide feedback to residential staff regarding EOC issues and concerns.
- Participates with ideas for improvement of services through a dialogue with supervisor.
- Utilizes the application of technology within operations and services to enhance quality, increase outcomes, and lower costs.
- Understands and adheres to AMHRS Medicaid requirements.
- Respond in a timely manner to all consumer’s questions and concerns.
- Must hold a current Registered Professional Nurse (RN) Licensure.
- Associate’s or Bachelor’s Degree in Nursing with prior experience in behavioral health.
- One to two years nursing experience preferably working with targeted population.
- Business operations experience required.
- Must hold a current CPR and BLS certification.
- Must have a valid NJ driver’s license.
- Must possess basic computer skills for electronic medical records(EHR) required.
