Summary of Responsibilities

PRN:
A hospice Registered Nurse provides care for patients with life-limiting illnesses in the patient’s home, assisted living communities, residential care facilities, nursing homes, hospitals, or anywhere the patient may call home. The Registered Nurse is responsible for using their professional skills to plan, coordinate, provide, and document care for terminally ill patients to ensure all physicians’ orders and the patient’s specific needs and end of life requests are fulfilled. The Registered Nurse may develop and implement a hospice plan that meets each to maintain dignity of the patient and their family during difficult and emotional times. Our mission is to improve the quality of life through compassionate care. Our vision is to allow those that have hospice to remain independent as long as possible.

On-Call:
The Hospice On-Call RN provides skilled- nursing care to our patients over the phone and in person as necessary. The main duty of the Hospice On-Call RN is to answer calls from the patient, caregiver, and/or family member and use critical thinking to determine the issue/need to curate the best plan of care that maintains all physicians’ orders and the patient’s comfort, quality of life, and specific end of life requests. This could include providing education over the phone patient/ family/ caregiver, coordinating with case managers for follow up visits, and/ or initiating an intervention in person. Qualifications

• Graduate of an accredited Bachelor of Nursing or Associates of Nursing program.
• Currently licensed as a Registered Nurse in the state of Idaho or other compact state.
• One (1) years of registered nursing experience in a community/home health or medical/surgical experience is preferred.
• Obtain and maintain current BLS CPR.
• Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
• Access to a well working automobile for traveling to patients’ home to provide care.
• Licensed driver with an automobile that is in good working order and insured in accordance with agency requirements.
