Requirements:
- Registered Nursing License in the State of Wisconsin
- Good communications skills
JOB DUTIES AND RESPONSIBILITIES:
1. Evaluates all hospital referrals, coordinates admission process and completes all admission documentation.
2. Assess and evaluate information provided by the DIB, Charge Nurses and various other departments contributing to the residents' MDS assessments.
3. Monitor care related information and lab results that influence resident acuity, treatment, diagnosis, and classification and make changes in the MDS as appropriate
4. Assure resident care plans are completed and updated timely in compliance with Medicare/Medicaid rules and regulations.
5. Coordinate the assessment schedule with the inter-disciplinary team to ensure timeliness of assessments and maximize reimbursements for all Medicare PPS assessments.
6. Maintains compliance with requirements in compliance program/plan.
