Hospice El Paso is the only non-profit hospice organization in the city. We are the oldest (40+ years) and largest hospice in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort and support to persons with a terminal illness, regardless of their ability to pay.
Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones. Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains; plus admissions counselors and support staff.
The majority of these services are provided at home. Hospice care can also be provided in nursing, foster, and assisted living homes; along with rehabilitation centers and skilled nursing facilities.
The “Center for Compassionate Care” is the city's first stand-alone “hospice in-patient facility.” We provide acute hospice care in a quiet and comfortable setting. In this home-like setting, patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment.

As a Case Manager, you will provide professional nursing care, as prescribed by the physician, in compliance with applicable RN licensure and agency policies and procedures, to patients who are receiving hospice care.

Minimum Requirements:
· Current licensure as a Registered Nurse in the State of Texas.
· Two years clinical experience. Background in ICU, oncology, discharge planning, care of terminal patients or community nursing health preferred.
· Personal vehicle available for transportation during work.
· Valid driver’s license and insurance as required by the State of Texas.
· Must be able to read, write, and speak English at a level which will permit the employee to accurately understand and communicate information as needed to safely and efficiently perform job duties.
· Bilingual in the Spanish language preferred.
Duties and Responsibilities:
1. Performs physical, psycho-social and spiritual assessment for patients at the start of care and establishes a plan of treatment in cooperation with the remainder of IDT members.
2. Performs intermittent skilled nursing evaluation on assigned patients
3. Assesses and documents current pain level and outcome with every visit.
4. Instructs patient’s family and caregivers as appropriate to overcome identified knowledge deficits.
5. Reports changes in patient’s condition to Team Leader daily and physician as appropriate.
6. Obtains physician orders and implements interventions and changes in treatment as appropriate.
7. Monitors medication levels at each visit, ensuring the patient always has sufficient quantity, but adjusts levels to coordinate with patients declining status.
8. Makes appropriate referrals to other disciplines, recognizing need for additional assistance.
9. Makes visits at the time of death and pronounces death as authorized by federal and state guidelines.
10. Provides nursing services in Admissions and at the in-patient unit as needed.
11. Provides instruction, leadership, and direction to LVNs as needed.
12. Provides supervision, instruction, leadership, and direction to CNAs
