Job Description

Collaborate with WMG HVSL practices to ensure patient referrals are paired with the right provider in the appropriate amount of time. Requesting candidates with a Process Improvement background, proficient in Microsoft applications (word, PPT, Excel). Excellent communicator (especially with physicians), self-starter and ability to work independently.

Duties & responsibilities

1. Reviews all unique referrals (internal, external, self), medical information and imaging studies. Based on clinical assessment (may include patient contact) determines appropriate first provider appointment and time frame for Heart and Vascular appointment.

2. Manages and coordinates work queues of referrals/follow up in Epic. Ensures timely first touches and other quality metrics. Monitor referral WQs to ensure timely appointment scheduling to avoid delays in care.

3. Identify and recommend necessary information (studies, medical records) to assist patient for appointment readiness with right provider and right time.

4. Relays information and recommendation to scheduling staff, uptrains new and established team members on changes or additions to staffing. Create educational tools to up-train staff members to make appropriate referral destination decisions.

5. Prioritizes urgent/emergent patients with Cardiology and triages appropriately.

6. Accurately reviews and consults with physician/APC regarding imaging and cardiac work up studies, as necessary.

7. Assists and guides physician/APC with insurance denial process, including appeal letters Provides guidance for non-par insurance situations.

8. Reviews daily physician/APC schedule, identifying openings for rapid triage and fast pass promotion.

9. Collaborate with WMG site-directors and subspecialty care coordinators to develop protocols for HVSL specialties to promote standardization across the system.

10. Serve as a liaison between Clinical and Practice Managers within the HVSL WMG practices to identify gaps, drive systemization and promote our clinically integrated network.

11. Collect data on referral patterns to be used to drive decision making at HVSL senior leadership level. Ensures that post – procedural and ED protocol patients are meeting scheduling expectations.

12. Coach referring practices on appropriate use of referrals and necessary pre-appointment testing if needed.

13. Engages the Contact center collaboration and for decision tree development and standardization.

14. Optimize EPIC referral operations with appropriate build teams and provide real-time process improve for referral routing issues.

15. Ongoing professional development in performance improvement, interactions with multidisciplinary partners, educational reference/material.

16. Create standard work, proactively connect with physician leaders, implement, and lead performance improvement projects as needed.

17. Seek feedback from HVSL physicians on the appropriateness of their schedules and makes effort to develop materials that guide to our mission of right patient to right provider at the right time.

Required Education & Experience

Current RN license in PA

Medical office experience is preferred

Supervisory experience is a plus
