Overview:
Fairview Range is looking for a Registered Nurse to join our primary care team at the Fairview Mesaba Clinic in Hibbing, MN. We provide high quality health care for the entire family from newborn to geriatric, with the full range of primary and specialty care services. With clinics located in Hibbing, Mountain Iron, and Nashwauk, we provide convenient and easy access to our care services. Through our relationship with M Health Fairview, your local primary care provider can extend direct access to the physician specialists and advanced services offered by University of Minnesota Physicians to you and your family.

The RN Care Coordinator focuses on the needs of the patient and their family as they access care from the health care home. The RN serves as a resource and educator to the patient and their family, and coordinates care efforts for a safe, effective, efficient and patient centered transition along the health continuum. The RN provides pro-active disease management using evidence based practices; acts as a liaison between patient/family, medical staff and others involved in treatment (ie ER, Hospitals, nursing homes, home health care and the community at large) promoting patient/family-centered care based on jointly determined health care goals and priorities as defined in the individual care plan; and coordinates implementation of health care home improvements (patient registry, care planning, health outcome data management). As a member of the care team, the RN Care Coordinator supports quality patient and family-centered care principles through performing a variety of high level functions within a care team to support the needs of the healthcare team and patients/families. This position will be primarily based in Hibbing, but may provide coverage in Nashwauk and Mountain Iron as needed. This position is full time, 72-80 hours per two weeks, 8-hour Day/Evening shifts and weekends as needed.

Responsibilities/Job Description:
  • Identifies particular needs and implements quality standards of nursing care in area of clinical specialty.
  • Evaluates and documents status and follows patient progress through phone calls, test results and report preparation. Evaluates quality of nursing care via patient rounds.
  • Serves as a liaison between patient/family, medical staff and others involved in treatment, promoting patient centered care based on jointly determined health care goals and priorities
  • Provides direct patient care in the area of specialty.
  • Functions as a resource to patient care units, outpatient clinics and medical staff regarding the clinical specialty.
  • Provides education services to patients and their families and other hospital staff regarding the clinical specialty.
Qualifications:
Required:
  • Care Coordination experience.
  • Specific job requirements will be based on position needs for specialty areas such as anticoagulation, and chronic diseases.
  • Graduate of an accredited school of nursing.
  • Current Minnesota license to practice professional nursing.
Preferred:
  • Three years clinical nursing experience.
  • Two years of specialty experience doing care coordination.
  • Baccalaureate Degree in Nursing or evidence of intent to acquire within two years of hire.
  • Health Care Home experience.
