SCHEDULING COORDINATOR I
Job Summary:

The Scheduling Coordinator I, maintains all clinical field staff schedules by coordinating appropriate visits to include: admissions, discharges, resumption of cares, recertification visits, and routine follow up visits on a daily basis. These tasks are managed through the workflow in our EMR System Homecare Homebase (HCHB), and through continuous communication with the clinical field staff.

Responsibilities:*

  • Manages all aspects of the clinical field staff schedules on a daily/weekly basis while ensuring admitted patients are seen as ordered by the referring physicians.
  • Compares, monitors, and makes any necessary changes to the actual/planned weekly visit schedule in Homecare Homebase (HCHB) scheduling console.
  • Manages all Rescheduled / Reassigned / Declined visit requests within a timely manner based upon the allotted time frame.
  • Communicates with clinicians continuously through phone calls, messaging & email.
  • Adjusts schedules as needed for any unexpected call-outs to cover all visits.
  • Refers patients to outside vendors as needed to staff visits or provide services.
  • Regularly assists the clinical field staff in meeting productivity expectations by adjusting the schedule as needed.
  • On an on-going basis, ensure all new patients are assigned to a case manager and are scheduled accordingly.
  • Schedules all disciplined staff evaluations with the appropriate field staff within 5 days of SOC or based upon appropriate visit utilization standards.
  • Reports to the Scheduling Coordinator II whenever it is not possible to get visits covered after all other means have been exhausted.
  • Assembles all disciplined staff schedules prior to the upcoming work week and makes needed changes as necessary before 5 PM on a daily basis.
  • Ensures compliance with all State and Federal regulations/ practices.
  • Attend and participate in all agency wide meetings/ in-services related to the Scheduling position as appropriate.
  • May perform other duties as assigned by management staff.

From date of hire, employee shall begin annual accrual of 10 vacation days, 6 sick days, and 9 holidays to be used as scheduled/needed

Qualifications:

High School Diploma or Equivalent.

  • 1 to 2 years of experience in a Home Health Scheduling setting preferred.
  • Must have professional and customer-service-driven phone and communication skills.
  • Ability to communicate effectively and tactfully with management, clinical staff and office staff in a constructive, goal directed, and professional manner.
  • Ability to communicate effectively and tactfully with patients, doctors, and all customers of Lorian Health (LORIAN) in a productive, constructive, and professional manner.
  • Strong ability to organize and prioritize workload on a regular basis based on the needs of LORIAN patients and the Patient Care Coordination department.
  • Ability to be flexible and to follow verbal and written instruction in a fast-paced team-oriented environment.
  • Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.
  • Ability to present reports to small and large groups upon request.
  • Strong knowledge of Microsoft Office Systems (i.e. Word, Excel, PowerPoint, etc.) and computerized systems in general required
  • Must be able to type at least 35 wpm
  • Ability to develop and maintain EMR.
  • Participates in maintenance of quality, safety, and/or infection control standards.
  • Working knowledge of Medical terminology.
  • Must maintain confidentiality of records and information at all times.

Job Type: Full-time

Pay: $20.00 - $28.00 per hour
