Social Work Care Manager
VillageCare is looking for a self-motivated and passionate Social Worker for our Full Time Care Manager position, who will provide telephonic and in-home assessments of member's status and needs. This position works with our patients to develop, implement, and monitor their care plan and ensure continuity of care while monitoring long term care needs. This is a work from home position with field visits. You must be able to do field visits within the greater NYC area.
Some of your daily activities will include providing all aspects of care management such as planning, coordination, and assessment of health, environmental, and psychosocial status of the members on an ongoing basis. You will Identify health, environmental, and psychosocial risks for assigned members and recommend appropriate interventions. You will provide an on-going assessment of the effectiveness of the Member's plan of care.
We would like to speak to those who Minimum formal education of Master's Degree and a current NYS LMSW license. Minimum of one (1) year job-related experience in MLTC preferred. Bilingual preferred.
