JOB TITLE: Case Manager
SCHEDULE: Tuesday-Saturday (8am-4pm)
STATUS: Non-Exempt
REPORTS TO: Clinical Supervisor
JOB DESCRIPTION:
The Case Manager will be responsible for providing services to clients with a primary diagnosis of chemical dependence. The case manager is expected to cooperate as a team member that brings administrative, clinical and evaluative activities that bring the client, treatment services, community services agencies, and other resources together to focus on issues and needs identified in the treatment plan. It involves collaboration with the client and significant others, coordination of treatment and referral services, liaison activities with managed care systems, client advocacy and ongoing evaluation of the treatment progress and client needs. The Case Manager is responsible to the Clinical Supervisor.
QUALIFICATIONS:
Minimum Qualifications – RADT-1 or higher, or equivalent.
Preferred Qualifications – Degree from an accredited college in the behavior health field and 6 months experience working in the field of addiction and mental health.
- Ability to perform duties according to TAP21 “Addiction Counseling Compentencies” and “12 Core functions of the AOD Counselor” with respect to screening, assessment, individualized treatment planning, crisis intervention, group and individual counseling, case management, referral, discharge summaries, clinical documentation, and professional ethics.
- General understanding of American Society of Addiction Medicine (ASAM) levels of service and treatment and the Diagnostic and Statistical Manual of Mental Health Disorders (DSM-V).
- Ability to establish and maintain cooperative working relationships with subordinate staff, supervisors, utilization review, managed health care clinicians, medical and mental health professionals.
- Ability to positively interact and develop rapport with clients and their families.
- Possess current First Aid and CPR certification.
CASE MANAGER RESPONSIBILITIES:
- Assists clients with intake, assessment, and orientation to program upon admission or transition in care level.
- Conduct prescreening, biopsychosocial, and other assessments and develop clinical summaries, and narrative reports.
- Develop individualized treatment plans based on prioritized problem lists.
- Identifies problem areas that may impact clients’ ability to successfully complete treatment. Guides clients to develop strategies to overcome obstacles to success, and achieve completetion of treatment.
- Provide indivdual, group, and family counseling services while applying evidence-based practices; in coordination with other team members and within your scope of practice.
- Provide therapy services in an individual, group and family setting while applying evidence-based practices, and within your scope of practice.
- Delivers services in outpatient, residential treatment setting or in the community depending on assignments and client need.
- Facilitates family invovlement in client’s progress, works to improve family communication, and assists the family in working with clinical staff toward treatment goals.
- Evaluate and document client progress toward treatment goals and plan accurately and completely within set time frames.
- Develop and provide solid discharge plans and summaries for assigned caseload of the program ensuring all outside follow up appointments with physicians, therapy sessions, outside meetings and support groups are attended.
- Perform case management services for clients including referral to outside providers (dental, legal, psychiatry, medical, etc.) during clients treatment episode and as a part of aftercare planning.
- Provide appropriate treatment interventions relative to the specific needs of each client.
- Records all interventions and interactions with clients.
- Provide accurate assessment and referral when appropriate.
- Document clinical services accurately and completely within set time frames.
- Provide effective treatment service as reflected by the client meeting his/her individual goals.
- Cooperate with Utilization Review when necessary in providing continued stay clinical justification.
- Seek clinical consultation throughout each client’s treatment episode and when encountering new and/or high risk clinical circumstances.
- Maintains frequent communication with the treatment team and engages in regular consults, clinical staffing, case conferences, and plan development meetings.
- Participate in the agency’s Orientation training, staff meetings, and in-service training.
- Participate in the agency’s Quality Management system.
- Adhere to ethics, legal aspects of clinical practice, and professional standards.
- Maintain compliance with HIPPA regulations, ADA, DHCS, CARF, and all ATCR policies and procedures.
- Maintains knowledge of new developments in the field of substance abuse.
- Exercise awareness in cultural competnecy issues.
- Conduct other tasks as needed or assigned.
