CAP is excited to announce that we are currently hiring a Medical Case Manager II. Responsibilities include: screening clients for psychosocial needs, developing goal plans, implementing effective interventions and monitoring client progress; assisting clients with applications for insurance, public assistance benefits, etc., and monitoring the process from initiation through formal decision; providing information and referral to a wide range of community services; providing advocacy within multiple settings including educating clients to self-advocate when appropriate; providing problem solving interventions as needed to assist client’s completion of their goals; working closely with providers and inter-disciplinary team to facilitate client care and resolve short-term needs.
COMPENSATION: $24.65 hourly, non-exempt
NOTE: This position is part of a union-represented bargaining unit. The compensation, benefits, and conditions of work for this position are collectively bargained. This is a full-time (40 hours per week), non-management position.
Successful candidates will have:
- Prior experience providing case management services with knowledge of care coordination, client advocacy, and community service navigation
- Experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction

MINIMUM QUALIFICATIONS:
- Bachelor’s Degree in the social services field (Social Work, Psychology, Public or Community Health) with two (2) years’ professional experience providing direct case management services OR minimum of three (3) years’ professional experience providing direct case management services
- Prior experience providing case management services with knowledge of care coordination, client advocacy, and community service navigation
- Proficiency with tech and tools and software, including Microsoft Office (Word, Excel, PowerPoint, Outlook) and willingness to learn new software
- Experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction
- Strong understanding of issues relevant to gay/bi/trans men, people living with HIV and AIDS, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery
- Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings, and critical discussions for how CAP can better serve the communities that we serve
- Demonstrated ability to effectively collaborate with community stakeholders
- Valid driver’s license and access to a reliable vehicle
- Available to work occasional evenings and weekends
PREFERRED QUALIFICATIONS:
- Master’s degree in the social services field (Social Work, Psychology, Public or Community Health) OR minimum of four (4) years professional experience providing direct case management services
- Verbal and written fluency in English and Spanish
- Experience working with people living with HIV/AIDS (PLWHA) and/or people with disabilities, chronic illness and/or other barriers to employment
- Lived experience of systemic oppression and non-dominant cultures. We encourage applications from Black, Indigenous, and other people of color; transgender, non-binary, and other LGBTQ+ people; and people with disabilities, including HIV.
- Demonstrated commitment to advancing equity and inclusion in workplace or community settings
- Knowledge of health literacy and navigating health care and health insurance systems in Washington
- Knowledge of social services
