Responsibilities:
- Creates Care Plan for clients via personal visits to assess patient requirements for care. - Conducts on-going assessment of patients
- Revises care plan as needed.
- Records all clinical and progress notes and enters them into the patient’s permanent record files
- Responsible for overseeing any medical issues as it relates to clients and staff. Experience/Qualifications:
Requirments:
- RN license active and in good standing.
- Minimum of 1 year supervisory experience
