General Description:
The Wraparound Facilitator provides Wraparound Facilitation for eligible youth and families; facilitates Wraparound Teams to develop Plans of Care; works collaboratively with community partners to arrange needed supports; coordinates and collaborates with system of care/community partners. This job is full-time, and the typical schedule is Monday-Friday from 8:00 AM to 4:30 PM.

Salary: $18-$19 Per Hour
Essential Functions:

  • Maintains a caseload of up to 15 youth. Maintains records of various Wraparound Team documents.
  • Maintains accurate and detailed administrative records.
  • Facilitates Wraparound Team Meetings, and helps develop, and implement individual Plans of Care with the team.
  • Works with CCO Care Managers to plan and coordinate the activities of wraparound service with the mental health providers and system of care.
  • Works with other youth serving agencies to implement coordinated and effective service provision to youth and their families.
  • Works with families to build informal networks and social supports including engaging with child and family advocacy programs.
  • Advocates as appropriate on behalf of families for needed supports from system of care community partners.
  • Provides information on and referrals to specialty services; arranges for a variety of goods and services to meet the needs of a youth or family as identified by the Wraparound Team including but not limited to respite care; medical; social or recreational services; etc.
  • Maintains and increases knowledge through reading, attending a variety of training programs and conferences, and other resources.
  • Adheres to Oregon Administrative Rules pertaining to Wraparound Care Coordination as required by the State of Oregon (OAR 309-019-0326).
  • Maintains a flexible work schedule, and travels to families’ homes and community locations when needed.
  • Provides case studies or progress reports as directed.
  • Performs other duties as assigned.

Minimum Position Requirements:

  • A Bachelor’s degree or combination of three years’ experience in a related field.
  • Must have Qualified Mental Health Associate (QMHA) certification or meet criteria to be credentialed as a QMHA by MHACBO
  • Direction Service is subject to the State of Oregon vaccine mandate for healthcare workers. New hires must submit vaccination documentation showing they are fully vaccinated, or be approved for an exception/accommodation, by their hire date.
  • Driving is an essential function for this position. Must possess a valid driver’s license and an acceptable driving record. Employee is required to maintain insurance on their vehicle in accordance with Oregon law.
  • Employment contingent on a satisfactory Criminal History Check as required by the State of Oregon (OAR 407-007-0200 to 407-007-0370) and as deemed acceptable by Direction Service.

Preferred Qualifications:

  • Bi-lingual in Spanish. Spanish speakers will recieve an increase in starting salary.

Physical Requirements:

  • Must be able to work in an office or outdoor environment including sitting for extended periods of time, standing, walking, bending, twisting, reaching, balancing, hear and/or respond to verbal/audio cues, see and/or respond to visual and read computer screen. Must be able to lift up to 25 pounds at times.
  • Exposure to communicable diseases found in children and the public
  • Ability to cope with clients who may use challenging or offensive language with the potential for escalation to physical aggression.
